Per Capita Eligibility Dates
- If you were enrolled by any of these dates and have NOT claimed your per capita. Adult Verification and W9 forms are available at the Enrollment office, per capita forms can also be mailed/faxed out upon request or you can print out forms under Enrollment Forms.

Deadline for BIA Land Judgement Payment
July 3, 1983 Born By 01/1983
1st Per Capita Deadline for (1997) $2000.00 Payment
Must Be Enrolled By – June 27, 1997
2nd Per Capita Deadline for (1999) $2000.00 Payment
Must Be Enrolled By – December 26, 1999

For 18 YEAR OLDS CLAIMING PER CAPITA
- 18 Year old claiming Per Capita must fill out all forms upon their eighteenth birthday or thereafter which are the Adult Verification form and the W9 form. If received prior to eighteenth birthday, form(s) will be voided and another set of form(s) will be sent out, this may cause a delay in per capita payment.
- All original form(s) along with a copy of Social Security Card Must be submitted to the:
