Become A Provider

A Child Care Provider offers child care assistance/subsidy to families who are working or in school by assisting with child care services.

How it works:

• We are similar to D.E.S. Child Care Subsidy.
• Centers/Group Homes/Home Providers bill us monthly for approved child care services. No paid absences.
• Parents are responsible for a portion of the cost in the form of a co-payment paid directly to the provider plus any additional costs if prices are above our approved rates.

Documents needed to become a provider:

1. Complete Provider Application.
2. TON Transaction Privilege Tax License ($5 for 5 years).
3. DHS or DES License (if applicable)
4. Az Dept. of Public Safety Fingerprint Clearance Card
5. CPR/FA certification and Food Handlers Card
