To start please complete our Referral form.
The application process includes:
- Complete the Referral Form and return it to the Vocational Rehabilitation Program
- A member of VR team will reach out to you to discuss the program, if appropriate for you, you can submit an application.
- Initial interview with a VR Specialist to Determine eligibility
- Development of an Individualized Plan for Employment
- Begin with Vocational Rehabilitation Services
