Elders Assistance Registration

The Division of Senior Services shall not be held liable for items lost, stolen or damaged during the Elders Day Event. The Division will not be responsible for medication administration or any person’s failure to bring medication with them to the Elders Day Event. Participants assume all inherent risk of injury and agrees to release and hold the Division harmless in the event of an injury or death.

Information contained within the Pre-Registration Form and On-Site Registration Form is the property of the Division of Senior Services; I give the Division of Health & Human Services my permission to use information gathered on this form for Data/Grant purposes. All photographs taken of me during the 27th Annual Elders Day Event are the sole property of the Division of Senior Services to be used in accordance with the mission and purpose of the division

By signing/registering for this event I give permission to video record my image likeness, those video recordings become the property of TONCA TV.
