New EAP Applicant Requirements:
- EMPOWER Eligibility Form
New Student Application – Applications are currently closed. - Official Transcripts
- High School Graduates/GED
Must submit official transcripts from high school with posted graduation date or GED scores - College/Some College
Submit official transcripts or e-scripts from the most recent college &/or university attended.
Note: Official transcripts must be certified by the school’s registrar’s office and either mailed directly to the program, emailed (e-script), or hand-delivered in an original, unbroken, sealed envelope.

Provide a letter of acceptance from the institution you will be attending.- Part-time – 6-11 credits
- Full-time – 12+ credits
- Class Schedule
Provide the class schedule from your institution. This will determine your status as a full-time or part-time student. Ensure that the number of credits aligns with those listed on your Financial Needs Analysis (FNA). - Program of Study
Provide a document that outlines the required courses for completing your program or degree. You can obtain this from a school advisor or use the Degree Audit available in your student portal. - FAFSA Submission Summary (formally known as SAR)
This is the report generated from the information you submit on your FAFSA. Complete and submit the Free Application for Federal Student Aid (FAFSA) online at www.fafsa.ed.gov. - Financial Needs Analysis (FNA)
This form, included in your packet, requires you to complete the top section. Your financial aid officer at the school you are attending will complete the bottom section of the form.- The form cannot be finalized until you have applied for Pell (FAFSA).
*Note Student is to fill out the top portion of the FNA and submit to school’s financial aid office for completion. - Students will need to contact the school for process of submitting to financial aid office.

All recipients and schools must complete and sign a W-9 Form. This form is required by the Tohono O’odham Nation to process payments to anyone potentially receiving funds.- It is a requirement for all students, seeking eligibility for assistance.

- Application, Goals Page and Signed Agreement
The agreement is digitally signed by the applicant/recipient, by signing the applicant agrees to adhere to the terms, policies and protocol set in place for the Education Assistance Program.
Intake timeline – To determine your status, all documents must be completed and verified.
EAP Intake Team will communicate through numerous emails, letters and/or phone calls for the status of your application.
