If you have interest in enrolling your child in the Head Start Program please complete the Head Start
Application.

Completed application packets include the following documents:

• Birth Certificate
• Income Guideline Form
• Income Verification Form
• Proof of Income
• Parental Consent for Health Services Form
• Health History- General Health History Form p.1 and p.2
• Tribal Enrollment Letter
• Immunization Document
• Guardianship Document
