How to Apply

To start please complete our Referral form. The application process includes:

1. Complete the Referral Form and return it to the Vocational Rehabilitation Program
2. A member of VR team will reach out to you to discuss the program, if appropriate for you, you can submit an application. 
3. Initial interview with a VR Specialist to Determine eligibility
4. Development of an Individualized Plan for Employment
5. Begin with Vocational Rehabilitation Services
