Applying for the Education Assistance Program (EAP)

Fall Semester: July 15, 2024
Spring Semester: January 4, 2025

Documents Required for New EAP Applicants:

1. Application, Goals Page and Signed Agreement
    Signed between the Recipient (you) and the Tohono O’odham Nation Education 
    Assistance Program.

2. Tribal Enrollment Status Form
     Included in packet of documents. Must fill out and submit to Tohono O’odham Nation 
     Tribal Enrollment Office.

3. Official Transcripts
    High School Graduates/GED
    Must submit official transcripts from high school with posted graduation date or 
    GED scores

    College/Some College
    Must submit official transcripts or e-script for any college or university last attended.
     * Official Transcripts are certified by schools registrar’s office and either, mailed directly to 
        the program, emailed or hand delivered, in original, unbroken, sealed envelope.

4. Acceptance Letter
    Letter of acceptance from the institution you will be attending.

5. Class Schedule
    The Class Schedule from the institution you are attending. This determines if you are 
     a full time or part time student.

6. Program of Study
    A document listing the required classes you need to complete your program/degree. 
    You will need to see a school advisor to obtain this or you may use your Degree Audit 
    in your student portal.

7. Student Aid Report (SAR)
    The report generated from information submitted on your FAFSA. Complete and 
    submit a Free Application for Federal Student Aid (FAFSA). Please apply at 
www.fafsa.ed.gov online.

8. Financial Needs Analysis (FNA)
    This document is a form in your packet that you need to complete the top portion. 
     Your financial aid officer will fill out the bottom portion of your form.  
      •  This form will not be completed until you have applied for Pell (FAFSA).  
      •  You have 2 files you must complete, one is for EAP and the is for your school.

9. W-9 Form
    All recipients must complete and sign a W-9 Form. This form is required by the 
    Tohono O’odham Nation in order to process payment to anyone receiving fund from 
    any of its entities.
